This form must be submitted within 24 hours of booking a training session or package. If you fail to do so your time slot may be forfeited. 

 

Your $50 deposit is non-refundable and will contribute to the total cost of your training session. The remaining balance for In-home sessions & Day Training is due the day of your session via cash, check, Venmo, or Zelle. The remaining balance for Packages & Day Camp must be paid in full on the first day of training. In the unfortunate event that something comes up and you need to reschedule or cancel your appointment please let me know within 24-48 hours PRIOR to your session.

Any request to reschedule your session needs to be made within this time frame or your deposit will be forfeited.
